How do you know a chronic disease is a disability?

From the way I see it, a chronic disease becomes a disability when it significantly affects your way of life forcing you to make significant changes to doing things unlike before. This means a total shift to the way you knew life before the illness, or before the illness advanced to take a toll on its victim.
